Online Student Profile Form
Creating the ideal lesson environment. Creating a successful teacher/student relationship is very important to us. Our faculty members have many specializations, diverse talents, teaching styles and personalities. The more we know about the way you or your child learns, personal goals and interests, the more likely that we will create a lasting and rewarding partnership. Your first step is to fill out this profile form. The Music School Registrar will receive your form, and after careful review, a member of our admissions team will contact you to discuss placement with a Music School faculty member. (Paper copies of this student profile form are also available at the front desk of each branch.)

Youth Ensembles

Online Audition Form
Our Youth Ensembles program includes youth orchestras, wind ensembles, jazz ensembles and chamber music ensembles. Acceptance and placement in an ensemble is by audition. Sue Curzio, the Youth Ensembles Coordinator, will review this form, and she will contact you to set up your audition day and time. (Paper copies of the student profile form are also available at the front desk of each branch.)

Early Childhood Classes

The first step to enrolling your child in our Early Childhood program is to fill out the student profile form. The Music School Registrar will receive your form, and a member of our admissions team will contact you to discuss placement into an Early Childhood class.
